Adoption Agreement

Please think carefully about this adoption. A Golden Retriever usually lives to the age of 12-14 years. If you do not think you can commit to a dog for life, please reconsider this adoption.
Goldens shed! We suggest you brush your dog daily. Not only is it beneficial for your dog’s coat, but also it allows you to interact with your dog, and quickly become familiar with any changes your pet may experience.
LIABILITY: Many of our dogs have come from animal shelters or impound facilities. We may have no history of their past circumstances. We assume no liability for unknown historical information about any dog. We try to pass on any information known to us. We assume no liability for information given to us by a former owner or caretaker. We have clear title to the dogs we place, either through shelters or by relinquishment from a previous owner.
You agree to indemnify and hold us harmless for any occurrence known or unknown, anticipated or unanticipated, relating to any dog you adopt from us, its behavior, care, keeping or maintenance. From the time of adoption, you are the animal’s owner, and solely responsible for its behavior.
NAMES:We name each dog (or use their previous name if known) for record-keeping purposes. If you choose to change the dog’s name, we recommend that you choose something that sounds similar to the original name, and notify us of the name change. We require that all dogs adopted from us wear proper identification. (Name, number, address)
VACCINATIONS:We will provide you with a rabies certificate or tag, and microchip information. Please call the veterinary hospital that gave the rabies shot and have the rabies number changed to your name. If the dog becomes lost and is picked up by Animal Control without other identification, they will be able to trace the dog to you with the rabies tag number.
If you fail to call the veterinarian and we are called by any Animal Control to come pick up the pet and pay the impound fee, it will be at the discretion of Golden Retriever Rescue Southern Nevada, whether the dog will be returned to you with charges of not less than $25 mileage plus the impound fee, or retained for re-adoption.
The dog you have chosen has also been given the Parvo combo shot and the Bordetella vaccination. We will provide you with this information and the due dates of the next shots.
It is not cost effective for us to have each dog tested for heartworm or Lyme disease, because it is so rare in this area. Please discuss having your dog checked for heartworm or Lyme with your vet, and follow their recommendations.
RETURN POLICY: We will refund your adoption donation in full if you return this dog within 14 days of adoption. After that, you are no longer entitled to a refund.
If, at any time in the dog’s life you can no longer keep the dog for any reason,you must return the dog to us.If you want to give this dog to another person, this person must come to us to adopt the dog, if they qualify. Any costs incurred in retrieving a dog that has been given to any party other than Golden Retriever Rescue Southern Nevada will be billed directly to you and will become your responsibility. You will be required to reimburse GRRSN for those costs incurred.
DIET: Please give your new dog a good brand of dog food. Cheap brands are full of fillers that causes the dog not only eat more, but to have larger (and looser) stool volume. We recommend that you choose a premium brand of food. * Please contact your veterinarian if you have any questions about what types of food you should feed your dog.
LOST DOGS:If your pet becomes lost or missing, please notify us immediately. We have a good working relationship with most shelters as well as Animal Control officers and we can often help you find the dog.
MEDICAL RELEASE OF LIABILITY:Golden Retriever Rescue Southern Nevada will not be responsible for any medical bills incurred after adoption. We encourage donations to help us build up the medical fund.
AGREEMENT: I have read the terms of this agreement/contract and understand that Golden Retriever Rescue Southern Nevada cannot guarantee that the pet I adopted is a purebred, even with AKC certification.
I am at least 21 years of age. All of the information I have given is true and complete.
I understand that Golden Retriever Rescue Southern Nevada remains co-owner for the life of any Rescued Golden that comes through GRRSN’s program and that GRRSN has the right to reclaim any foster or adopted Golden if the Organization feels that doing so is in the dog’s best interest.
It is your responsibility to license your dog in the jurisdiction in which you live. The City of Las Vegas, No. Las Vegas and Henderson require your dog to be licensed. An application to license your dog will be added to your Adoption Packet. Golden Retriever Rescue Southern Nevada is a Nevada non-profit organization. Ask us for a receipt if you need one for tax purposes. We accept donations of all kinds (treats, blankets, toys, checks, etc.)
