If you have a Golden Retriever that you need to surrender, for any reason, please fill out the Owner Release Form below.  Our Intake Director will contact you immediately to arrange the intake.  If possible, prior to the surrender, please obtain copies of your dog’s most recent medical records from your current veterinarian that reflect your dog’s most recent vaccinations, microchip information, spay/neuter status, and any medical issues that your dog may currently have or has had in the past.
